Treatment of a 45-year-old patient whose religion does not permit transfusion of blood – a bloodless transplant

A 45-year-old American national, who is a Jehovahs’ Witness by religion, was treated in his home country for a mantle cell lymphoma and brought into complete remission. He was referred to HCG Hospital, Bangalore, INDIA for an autologous Bone Marrow Transplant, without using blood or blood products. 

The patient’s pretransplant was optimised with appropriate measures to maximize the hematological profile for three weeks, and he was then taken up for an autologous Bone Marrow Transplant. By day 7, after standard conditioning regime, which was managed without any blood products, he went into aneutropenia, severe anemia, and thrombocytopenia. His neutrophils were engrafted by day 10 and platelets showed delayed engraftment, warranting the need for agonist therapy. After a slow clinical recovery, he was shifted out of transplant and monitored. This was the first case of a bloodless Bone Marrow Transplant conducted in Asia.Thereafter, two more cases of bloodless transplant for Jehovah’sWitnesses have been conducted at HCG Bangalore. 3 years on, the patient is currently free of the disease, leading an active life.
